Portobello Medical Centre is an NHS general practice dedicated to providing the highest quality care to its patients, in a  comfortable purpose-built and well-equipped environment.

 

We are a small practice and offer a personalised care tailored to our patients’ medical needs. The practice team consists of trained medical staff – GPs, Nurses, HCA, Pharmacists as well as an administration team who manage the day-to-day operation of the practice.

Our practice has a GMS (General Medical Service) contract with Kensington & Chelsea Primary Care Trust and this allows us to focus on the delivery of good clinical care rather than bureaucratic procedures and  on improved professional development for our staff. As a PMS practice we aim to provide a multi-disciplinary team that can deliver a high level of medical service in a number of key areas including  cardiovascular disease, Stroke, Hypertension, Hypothyroidism, Diabetes, Asthma, COPD, Mental health, Cancer and Epilepsy.

We are part of a Primary Care Network whereby we work in collaboration with 9 other practices to meet the local needs of our patients.

West-Hill Health Primary Care Network

This practice is a member of the West-Hill Health PCN, which incudes Portland Road PracticeHolland Park Surgery, Pembridge Villas Surgery, Westbourne Grove Surgery, Portobello Surgery, Lancaster Gate Surgery, Garway Road Practice, Grand Union Medical Centre.

The PCN has a population of 67000 patients and our aim is to shape the services we deliver to the needs of our practice populations using a well-trained workforce to deliver good quality primary core services.

Improving Access to Your GP Practice

We’re committed to making it easier for you to get the care you need. As part of a wider North West London programme, our Access Improvement Plan focuses on delivering high-quality, timely, and personalised care. Here’s what you can expect:

  • Faster access to care: We aim to offer same-day or next-day responses for urgent medical needs and ensure more appointments are available, including evenings, weekends, and Bank Holidays. These additional appointments are delivered through our Enhanced Access Hubs, in partnership with the West London GP Federation.
  • Better support for patients with ongoing or complex needs: We prioritise continuity of care so you can see the same healthcare team when it matters most.
  • Smarter use of digital tools: The NHS App and our online consultation system (PATCHS) help you get support quickly and manage your health more easily from home.
  • We’re working to answer 90% of calls within 10 minutes: We regularly check our phone data to improve response times and reduce waiting.
  • More personalised help when you need it: Our trained reception team and Social Prescribers are here to guide you to the right care — whether that’s with a clinician or community support services.
  • Ongoing improvements based on your feedback: We monitor how we’re doing and regularly gather patient feedback to keep improving your experience.
